Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in Somerset, PA. This allows for a flexible schedule tailored to your preferences, offering the chance for supplemental income or community connection.

Nurse Practitioners will travel to patients' homes to conduct non-invasive assessments, ensuring accurate documentation and providing education on wellness. Ideal candidates hold a Master’s Degree and active NP license, with experience preferred but not required.

Qualifications

• Master's Degree OR commensurate experience and satisfactory completion of NP license.
• Active State NP license required.
• Active National board certification necessary.
• Current BLS certification needed.
• Preferred 12 months of experience as a paid advanced clinical provider.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
