Qualifications

• RN licensure in Rhode Island or through the Nurse Licensure Compact

• Specialty certification required
• Annual completion of mandatory infection control training, BLS, ACLS/PALS
• Prescriptive privileges per licensure

Responsibilities

• Oversee a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care
• Provide leadership in development and evaluation of standards, policies, procedures and critical paths
• Coordinate care across acute, home, and ambulatory settings
• Support healthcare goals through education and encouragement
• Provide direct care in acute, ambulatory and home care settings
• Perform advanced practice skills under Medical Director supervision
• Consult on clinical issues to enhance knowledge and skills of team
• Participate in education, clinical research and mentoring of providers
• Facilitate patient/family education in area of expertise
