Del Corazon Hospice in Santa Fe, NM is seeking a dedicated PRN Hospice RN to join our team. You will provide patient-centered, high-quality end-of-life care, collaborating with an interdisciplinary team to manage pain, symptoms, and comfort, while educating families. This per diem role offers scheduling flexibility and autonomy.

Requirements include NM RN licensure, a BSN (or equivalent), BCLS, and background checks. Experience in hospice, home health, or acute care is preferred.

Formación

• Graduate nursing program graduate required.
• Current NM RN licensure required.
• 1 year acute care/home health/community health/hospice experience in last 2 years preferred.
• Current BCLS certification.
• TB testing and pre-employment drug screen; criminal background check.
• Valid NM driver’s license and auto insurance.

Responsabilidades

• Assess patients’ physical, emotional, and spiritual needs.
• Provide direct patient care including wound management, medication administration, and symptom management.
• Collaborate with patients, families, and interdisciplinary team for coordinated care.
• Educate patients and families on disease processes, treatment options, and self-care techniques.

Job Summary

As a PRN Hospice RN, you will provide high-quality, patient-centered care to individuals with serious illnesses. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, manage pain and symptoms, and provide emotional and spiritual support to patients and their families. This is a per diem position, allowing for flexibility and autonomy in your scheduling.

Responsibilities

• Assess patients' physical, emotional, and spiritual needs

• Provide direct patient care, including wound management, medication administration, and symptom management

• Collaborate with patients, families, and interdisciplinary team members to ensure coordinated care

• Educate patients and families on disease processes, treatment options, and self-care techniques

• Document patient care and communicate with team members, physicians, and other healthcare professionals

Requirements

• Graduate of an accredited school of nursing.

• Current Registered Nurse licensure in the state of New Mexico

• Experience as an RN commensurate with one (1) of the following:

  • One (1) year acute care experience within the last two (2) years

  • One (1) year home health care experience within the last two (2) years

  • One (1) year community health or public health experience within the last two (2) years

  • One (1) year hospice experience within the last two (2) years

  • Current BCLS certification.

  • Current TB testing and pre-employment drug screening and passes a criminal background check.

  • Valid state driver’s license and reliable automobile.

  • Proof of current automobile insurance.

Hospice experience preferred.
