Job summary

MJHS Hospice and Palliative Care is seeking an experienced Nurse Practitioner to join our team. You will provide palliative care consultations across nursing homes, hospitals, and community settings, focusing on symptom management and goal-setting discussions.

The role requires an NP background, NY RN licensure, and ability to travel within the assigned geographic area. You will collaborate with a multidisciplinary team to develop and implement care plans and contribute to quality initiatives.

Qualifications

• Master's degree in Nursing is required.
• Graduate of an approved Nurse Practitioner Program is required.
• New York State RN licensure is required.
• CPR-BLS certification and NY Driver's license preferred.

Responsibilities

• Provide NP-driven palliative care consultations in nursing homes, hospitals or community settings.
• Develop and implement comprehensive care plans to alleviate illness burden for patients and families.
• Engage in goal-setting discussions, hospice eligibility reviews, and documentation in the medical record.
• Contribute to quality initiatives and educate staff to improve care delivery.
• Participate in provider on-call rotation and perform face-to-face encounters as needed.

The position will cover the following locations: Bronx, Upper Manhattan, and Isabella Nursing Home.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

The Nurse Practitioner (NP) provider is one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. The NP Provider may work in a nursing home, hospital or community environment. He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed.

• Master's degree in Nursing and Graduate of an approved Nurse Practitioner Program required.
• Minimum of 1-year clinical experience as a NP provider preferred. Hospice, Palliative Care and/or Geriatric nursing or NP experience preferred.

• New York State RN licensure, Certified Adult, Family, Gerontology, or Palliative Care Nurse Practitioner required.
• CPR-BLS and valid NYS Driver's license preferred.
• Hospice & Palliative Care certification strongly encouraged by year 2 of hire.
• Microsoft Word and Excel.
• Ability to work flexible hours to meet the needs of the position.
• Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
• Ability to travel within the assigned geographic area to provide patient care.
