Job summary

St. Croix Hospice in Huntley, IL is seeking a compassionate Nurse Practitioner to provide skilled medical care for terminally ill patients and guide families through end-of-life decisions.

You will oversee medications, recertifications, and care plans in collaboration with a multidisciplinary team. This full-time role requires NP licensure in the practicing state, a nursing background with at least two years of RN experience, and a commitment to HIPAA standards, autonomy, and patient-centered

Qualifications

  • Degree from an accredited NP program required.
  • Minimum two years of RN experience; hospice or palliative care experience.
  • Demonstrate HIPAA compliance and strong interdisciplinary collaboration.

Responsibilities

  • Provide comprehensive medical care for terminally ill patients, including assessments and care plans.
  • Prescribe medications and complete hospice recertification face-to-face assessments.
  • Collaborate with interdisciplinary teams and physicians to manage symptoms.
  • Educate and support patients, families, and caregivers on end-of-life care.

Through oversight of medications, treatments, and recertifications, the Nurse Practitioner helps ensure continuity of care, allowing families to focus on comfort, dignity, and meaningful time together.If you're a compassionate Nurse Practitioner who values meaningful patient connections and autonomy in your practice, we’d love to meet you.Nurse Practitioner Requirements & Key Responsibilities:Must have current Nurse Practitioner (NP) license in the state(s) of practice.Degree from an accredited NP program required.Minimum two years of RN experience; hospice or palliative care experience.Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.Benefits & Schedule:Schedule: 8:00am to 5:00pm M-FHealth, dental, vision, and life insuranceCompany-paid short/long-term disability and basic life insurance401(k) with up to 4% company matchTuition reimbursement and professional development opportunitiesPaid time off and paid holidaysThe starting salary range for this role is $120,000-$150,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
