Job summary

Pathways Healthcare is seeking a Nurse Practitioner to join our physician- and nurse practitioner-led home health and hospice team serving Worcester, MA and surrounding areas. You will conduct admissions, provide routine and urgent visits, and coordinate care with the team to support patient recovery at home.

Qualifications include an APA nursing master’s degree, APRN license, and 2+ years in home health. Compensation is per visit, with flexible scheduling and opportunities for professional

• APRN license (Adult/Geriatrics preferred) and DEA certificate.
• CPR certification and a Master's degree in Advanced Practice Nursing.
• Minimum 2 years as a Nurse Practitioner, home health care preferred.

Responsibilities

• Conduct admission assessments and create personalized treatment plans.
• Provide routine and urgent visits, flexible to patient needs.
• Manage medication orders and patient education on usage.
• Wound care, testing, and equipment/supply coordination.
• Document progress and collaborate with care team.
• Assist discharge planning and coordinate with community resources.
• Participate in team meetings to ensure cohesive care.
• Maintain patient confidentiality and report safety concerns.

Job Description:

• Service Area: Worcester, Sturbridge & Surrounding Areas
• Schedule: Flexible

Qualifications:

• Current APRN state professional license (Adult/Geriatrics specialty preferred), DEA certificate, CPR certification.
• Master's degree in Advanced Practice Nursing from an accredited school.
• At least 2 years as a Nurse Practitioner, preferably in home health care.
• Valid driver's license with reliable transportation.
• Excellent communication, problem-solving, and organizational skills. Homecare Homebase knowledge is a plus.
