Job summary

Midland Care Connection is seeking a Nurse Practitioner to provide primary care for older adults in home health, rehab centers, and long-term care settings. The role emphasizes geriatric and palliative care with a patient-centered approach.

You will collaborate with physicians, nurses, and a multidisciplinary team to develop care plans, manage medications, and support families in goals of care and comfort. Kansas license required.

Qualifications

• Current, unrestricted Nurse Practitioner license in Kansas.
• National board certification as a Nurse Practitioner.
• Graduate degree from an accredited Nurse Practitioner program.
• Current DEA registration and ability to prescribe medications.
• Current BLS certification.
• Experience providing direct patient care as a Nurse Practitioner.
• Strong assessment, clinical decision-making, and communication skills.
• Ability to work collaboratively as part of an interdisciplinary care team.

Responsibilities

• Provide comprehensive primary care to older adults in home health, rehab centers, and long-term care settings.
• Assess and manage acute and chronic conditions, monitor health changes.
• Develop individualized treatment plans and monitor recovery.
• Collaborate with physicians, nurses, families, hospice, home health, and rehab teams.
• Prescribe medications and manage complex symptoms.
