Our team focuses on comfort, dignity, symptom management, and quality of life while supporting patients and their families through every step of the journey. Through an interdisciplinary approach, Midland Care provides personalized medical, emotional, spiritual, and practical support in the setting that best meets each patient’s needs.
The Nurse Practitioner provides advanced clinical care to patients with serious or life-limiting illnesses, with an emphasis on comfort, symptom management, and quality of life.
The Nurse Practitioner evaluates patients, diagnoses and treats medical conditions, manages complex symptoms, prescribes medications, and develops or updates individualized care plans. They work closely with patients and families to understand goals of care, explain treatment options, and provide guidance throughout the hospice or palliative care journey. The Nurse Practitioner also collaborates with physicians and the interdisciplinary team—including nurses, social workers, chaplains, and other care professionals—to ensure care is coordinated and aligned with each patient’s wishes.
