Job summary
Upperline Health seeks a Nurse Practitioner for the Memphis, TN clinic to provide clinic-based and home-based care in collaboration with the local physician team. You will develop complex plans of care for adults with chronic conditions, conduct exams, and educate families on medications and nutrition.
Ideal candidates have boards in Adult/Adult-Geriatric/Family NP, DEA eligibility, BLS, and a track record in value-based care.
Qualifications
- Master’s degree in nursing is required.
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner is required.
- Value-based care experience preferred.
- DEA registration or eligibility is required.
- BLS certification is required.
- Experience providing care in patients' homes is required.
Responsibilities
- Develop and implement clinical plans of care for adults with chronic conditions.
- Gather history, physicals, and diagnostics in home or clinic.
- Educate patients and families on medications, diet, and prevention.
- Collaborate with interdisciplinary team to meet patient and family needs.
- Prescribe medication as permitted by the state Board of Nursing.
- Maintain patient medical records per state regulations and policy.
