Job summary
Upperline Health Inc. is seeking a Nurse Practitioner to provide clinic-based care in our Memphis, TN clinic, with responsibilities extending to home visits as needed within a value-based care model.
The ideal candidate has a Master’s in nursing, board certification in Adult/Adult-Geriatric/Family NP, and DEA registration or eligibility. BLS is required; home-based care experience is highly valued, with a collaborative team and growth opportunities.
Qualifications
- Master’s degree in nursing is required.
- Board Certification as an Adult, Adult-Geriatric or Family NP is required.
- Value-based care experience is preferred.
- DEA registration or eligibility required.
- Basic Life Support (BLS) certification is required.
- Comfort with home-based patient care in a fast-paced environment.
Responsibilities
- Develop and implement care plans for adults with chronic conditions in clinic or home settings.
- Gather patient history, perform exams, and order diagnostics.
- Collaborate with interdisciplinary team to address physical and psychosocial needs.
- Educate patients and families on medications, diet, and crisis prevention.
- Maintain accurate medical records and documentation per policy.
- Participate in weekly team meetings and state-regulated practices.
