Job summary

Vitae Health Systems is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.

The role offers full-time, on-site work in Georgia with a base salary range of $110,000–$130,000 and bonus opportunities.

Qualifications

• Master’s degree in Nursing from an accredited NP program.
• Active and unrestricted APN license in respective state.
• Current DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• 2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of chronic disease management and geriatric care principles.
• Ability to deliver value-based care focusing on outcomes, cost-efficiency, and reduced hospitalizations.
• Proficiency in evidence-based practice and clinical decision-making.
• Ability to work independently and collaboratively within an interdisciplinary team.

Responsibilities

• Perform comprehensive health assessments and manage acute and chronic conditions in a skilled nursing setting.
• Provide ongoing medical management including diagnostics, treatments, medication reviews, and monitoring.
• Deliver timely clinical interventions to improve outcomes and functional well-being.
• Support value-based care by focusing on quality outcomes and cost optimization.
• Collaborate with physicians and the care team to ensure coordinated care and participate in care plan meetings.
• Facilitate smooth transitions of care and reduce gaps in treatment during admissions/discharges.
• Educate residents and families on diagnoses, plans, medications, and preventive care.
