Vitae Health Systems is seeking a dedicated Nurse Practitioner to provide patient-centered medical care to residents in skilled nursing facilities. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.

The role offers a strong earning potential with a base salary plus uncapped bonus opportunities, plus a full benefits package and generous paid time off.

Qualificações

• Master’s degree in Nursing from an accredited NP program.
• Active NP license and DEA registration.
• Minimum 2 years clinical experience in geriatrics or long-term care.

Responsabilidades

• Perform comprehensive health assessments and manage acute and chronic conditions in a skilled nursing setting.
• Order and interpret diagnostics, prescribe treatments, conduct medication reviews, and monitor conditions.
• Collaborate with physicians and interdisciplinary team members to ensure coordinated care and participate in care plan meetings.
• Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies.
• Maintain accurate, timely, and compliant documentation and adhere to regulatory standards.
• Support value-based care initiatives and work toward reducing hospitalizations and costs.
