Job summary

Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in skilled nursing facilities. Qualifications

• Master’s degree in Nursing (required)
• Active and unrestricted APN license in the respective state
• Current DEA registration and prescriptive authority
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP)
• 2 years of clinical experience preferred, geriatrics/long-term care
• Strong communication and teamwork skills
• Proficiency with EHR systems

Responsibilities

• Perform comprehensive health assessments and manage acute/chronic conditions in SNF residents
• Order/interpret diagnostics, prescribe treatments, and monitor patient status
• Deliver timely clinical interventions to prevent hospitalizations
• Collaborate with physicians and interdisciplinary teams in care planning
• Educate residents and families on diagnoses, treatment plans, and preventive care
• Document accurately and comply with clinical/regulatory standards
