Job summary

Pride Health is currently seeking a Nurse Practitioner to work in Fayetteville, NC, providing specialized care at long-term care facilities. This position offers a contract of six months with opportunities for hire and a scheduled workweek of Monday to Friday, 8 AM to 5 PM.

The ideal candidate will have NP certification and at least one year of experience, especially in LTC or geriatrics. This role ensures comprehensive patient management and collaborative care with a focus on high-risk health plan members.

Qualifications

• Certification as Nurse Practitioner in one of the specified fields.
• Ability to provide specialized care.
• Experience working with geriatric populations preferred.

Responsibilities

• Provide specialized care to high-risk health plan members.
• Conduct comprehensive assessments for patient care.
• Work as part of an interdisciplinary team for care management.
• Manage acute and chronic medical and behavioral conditions.
• Ensure accurate documentation, including ICD-10 coding.
• Communicate with the care team for better patient outcomes.
• Facilitate discussions on advanced illness care planning.
• Offer counseling and education to patients and caregivers.

Skills

NP Certified in FNP, ANP, GNP, or equivalent
