Pride Health is seeking a Nurse Practitioner in Lillington, North Carolina. This contract-to-hire position involves providing specialized care to high-risk health plan members and managing both acute and chronic conditions. Successful candidates will have NP certification in FNP, ANP, or GNP, and at least one year of experience in long-term care, skilled nursing facilities, or geriatrics. Pride Health is hiring a Nurse Practitioner in North Carolina! (Contract to hire)

Schedule: Monday-Friday (8am-5pm)

Contract: 3-6 month, contract to hire

Pay Rate: $65-70/hour

Specific Duties

• Provide specialized care to high-risk health plan members
• Work as part of an interdisciplinary team
• Manage acute and chronic medical and behavioral conditions
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers
• No on-call
• No holidays required

Must have qualifications/experience

• NP Certified in one of the following (FNP, ANP, GNP)
• Access to transportation
• 1+ year of experience (LTC, SNF, Geriatric preferred)
