Job summary

Jobot is seeking a Nurse Practitioner to deliver primary care to seniors in a collaborative, patient-centered setting. Focus areas include chronic disease management, care coordination, and helping older adults stay healthy and independent.

The role offers a Monday–Friday schedule with no weekends or after-hours call, in Stockton, CA. Requires CA NP license, RN license, and DEA registration within 6 months.

Qualifications

• Active California NP and RN licenses.
• Graduate of an accredited NP program.
• Minimum of 5 years nursing experience.
• At least 1 year working with elderly or frail population.
• Experience in primary care, geriatrics, managed care, SNF, or home health preferred.
• DEA registration due within 6 months of hire.
• BLS certification required.

Responsibilities

• Perform initial, annual, and ongoing patient assessments and develop care plans.
• Manage acute and chronic conditions for a frail elderly population.
• Collaborate with physicians, nurses, and the care team.
• Educate patients and families on treatment plans and disease management.
• Coordinate specialty referrals, hospital transitions, and follow-up care.
• Maintain accurate and timely clinical documentation.
