Job summary

Premise Health in Lafayette, IN is seeking a Full Time Family Nurse Practitioner to provide primary care to client employees and dependents at our on-site Health Center. The role offers a Monday–Friday schedule with 4x10 or 5x8 shifts and hours 7:00 AM – 7:00 PM on a rotating basis.

The successful candidate will have state RN and NP licenses, national certification (ANCC/AANP), and 3+ years of ambulatory or family practice experience.

Qualifications

• Must hold state RN and Nurse Practitioner licenses.
• Active national certification (ANCC or AANP) is required.
• At least three years of clinical NP experience in ambulatory care, family practice or emergency department.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refers patients to specialists or physicians as needed.
• Maintains patient health records ensure accurate and up-to-date documentation and records.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.

As a Full Time Family Nurse Practitioner, you'll provide care to client employees and their dependents in our Health Center located in Lafayette, IN.

Position Highlights

• Full-time position (40 hours per week)
• Monday–Friday schedule with flexible options of 4 10-hour shifts or 5 8-hour shifts
• Clinic hours: 7:00 AM – 7:00 PM with a rotating schedule that includes two early shifts and two late shifts
• A patient-centered environment where providers have the time they need to deliver high-quality, personalized care—no rushed visits
• Strong focus on preventative care and long-term patient health management
• Patient visits commonly include annual wellness exams, preventative health screenings, and women’s health services
• Patient population ages 10 and up
• Primary care experience preferred
• Collaborative care team with strong clinical support

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state... (list continues)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
