Job summary

Apploi is seeking a compassionate Nurse Practitioner (NP) to provide primary and ongoing care to residents in a skilled nursing facility in New Jersey. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.

The ideal candidate holds an active New Jersey NP license and is board certified (FNP/AGNP/GNP preferred), with experience in SNF, long-term care, or geriatric settings.

Qualifications

• Active New Jersey Nurse Practitioner license is required.
• Board Certification as Nurse Practitioner (FNP, AGNP, or GNP) preferred.
• Experience in skilled nursing facilities, long-term care, or geriatrics is preferred.

Responsibilities

• Conduct routine and acute patient assessments for nursing home residents.
• Manage chronic conditions and develop treatment plans.
• Respond to changes in residents’ conditions and provide timely care.
• Order and interpret diagnostic tests and labs.
• Prescribe medications in accordance with New Jersey regulations.
• Coordinate care with physicians, nursing staff, and families.
• Ensure proper documentation in the EMR.
• Participate in care plan meetings and regulatory compliance.
