Job summary

Epic Healthcare in New Jersey is seeking a compassionate Nurse Practitioner to provide primary and ongoing care in a skilled nursing facility. The NP will work with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care on a daily basis.

This role requires an active NJ NP license and board certification (FNP/AGNP/GNP preferred) with experience in SNF, LTC, or geriatrics.

Qualifications

• Active New Jersey NP license required.
• Board Certification: FNP/AGNP/GNP preferred.
• Experience in SNF, long-term care, or geriatrics preferred.
• Strong clinical decision-making and communication skills.
• Ability to work collaboratively with nursing home staff.

• Conduct routine and acute patient assessments for nursing home residents.
• Manage chronic conditions and develop treatment plans.
• Respond to changes in residents' conditions and provide timely care.
• Order and interpret diagnostic tests and labs.
• Prescribe medications in accordance with New Jersey regulations.
• Coordinate care with physicians, nursing staff, and families.
• Ensure proper documentation in the EMR.
• Participate in care plan meetings and regulatory compliance.
