Job summary

Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will build relationships with residents, their families, facility staff, and providers, and play a key role in clinical leadership.

Our team emphasizes proactive geriatric care to prevent hospitalizations and improve outcomes in long-term care and hospice settings.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.

Responsibilities

• Ensure continuity of care with the primary In-House NP.
• Conduct new admissions evaluations for long-term residents.
• Perform comprehensive physical exams and monitor changes.
• Prescribe medications and treatments as needed.
• Develop care policies and coordinate referrals.
• Coordinate advanced care planning and discharge visits.
• Document assessments, treatment plans, and follow-ups timely.
• Support physician-patient relationship and address gaps in care.

As a Weekend In-House Nurse Practitioner, you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to maximize onsite time to develop robust caseloads
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:

• Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner
• Conducting new admission evaluations and monthly stability visits for long-term residents
• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care

Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
