Job summary

Cambridge Health Alliance (CHA) is seeking a Nurse Practitioner for the CHA PACE Program. The NP will provide high-value geriatric care with an interdisciplinary team, at CHA's PACE center in Cambridge or STR centers, ensuring care coordination and quality outcomes.

The role involves 36 hours/week, on-site coverage, and GoTo duties including calls from home care nurses and clinics. MA NP licensure and DEA are required.

Qualifications

• Graduate of an accredited Nurse Practitioner program with a Master's degree in Nursing.
• Current NP certification and licensure in Massachusetts.
• Massachusetts RN licensure required.
• Current Massachusetts licensure authorizing expanded practice as well as DEA number and Mass DPH registration to prescribe controlled substances.
• Minimum of 2 years of experience in geriatric health as an NP preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Provide care at short term rehab facilities or GoTo coverage as assigned.
• Work on site daily at the PACE center or at STR center as assigned.
• GoTo coverage may include triage calls, home care nursing, and same-day visits.
• Follow patients and provide care in the facility during STR coverage.
• Join a team in a unique healthcare model serving a complex adult population.
