Cambridge Health Alliance (CHA) is seeking a full-time Clinical Specialist II, Nurse Practitioner to join the CHA PACE Program in Cambridge. The NP will provide high-quality geriatric care across STR facilities and GoTo coverage as part of an interdisciplinary team.

Qualifications

• Graduate of an accredited Nurse Practitioner program with a Master’s degree in Nursing.
• Current NP certification and licensure in Massachusetts.
• Massachusetts RN licensure required.
• Current Massachusetts licensure authorizing expanded practice with DEA and Mass DPH registration to prescribe controlled substances.
• Minimum of 2 years of experience in geriatric health as an NP preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.

• NP works on site daily, either at our PACE center in Cambridge or at a STR center as assigned

CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE’s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services. We are seeking a dedicated and experienced Nurse Practitioner to join our highly functioning interdisciplinary team.
