Job summary
NeighborHealth is looking for a full-time nurse practitioner to provide primary care services to frail participants at the Neighborhood PACE of East Boston program based in Revere, Massachusetts. This independent provider will be part of a collaborative healthcare team, serving multiple geriatric settings.
The ideal candidate should have experience with frail adults and hold a MA Nurse Practitioner License. The position offers a competitive salary ranging from $135,000 to $164,564 based on experience with excellent benefits.
Qualifications
- 1 year relevant experience preferably in a community health setting.
- Ability to work with frail adults in various health care settings.
Responsibilities
- Obtain comprehensive and problem-focused health history.
- Perform comprehensive and problem-focused physical exams.
- Order and interpret screening and diagnostic tests.
- Communicate clinical findings to patients.
- Initiate consultations when necessary.
