Job summary

Matter Health seeks a compassionate Nurse Practitioner to deliver patient-centered primary and preventive care to older adults in affordable-housing communities around Springfield, MA. Care is provided in patients' residences, with telehealth as appropriate.

The role is full-time, field-based, requiring travel within the Springfield market to build meaningful patient relationships and improve health outcomes.

Qualifications

• National board certification in family, adult, gerontology, or adult-gerontology practice.
• Active and unrestricted Massachusetts NP license, or eligibility to obtain before employment.
• Active DEA registration and Massachusetts MCSR prescribing credentials, or ability to obtain them.
• Primary care, family medicine, adult health, gerontology, community health, or similar clinical experience preferred.

Responsibilities

• Conduct comprehensive patient assessments (history, physicals, diagnoses, ongoing evaluations).
• Develop and manage individualized, evidence-based care plans.
• Diagnose and manage acute and chronic conditions within scope of practice.
• Provide medication management, preventive care, health education, and timely follow-up.
• Identify barriers to care (social, environmental, access-related) and address them.
• Connect patients with community resources and support services.
• Collaborate with physicians and multidisciplinary care-team partners.
• Maintain accurate, timely clinical documentation.
• Deliver compassionate, culturally responsive care to build trust and engagement.
• Travel to assigned affordable housing communities throughout the Springfield market.

Matter Health is seeking a compassionate and experienced Nurse Practitioner to provide high-quality, patient-centered primary and preventive care to primarily older adults living in affordable-housing communities throughout the Springfield, Massachusetts area. Care is delivered directly inside patients’ residences, with telehealth services used when appropriate

This is a full-time, field-based opportunity for a Nurse Practitioner who wants to build meaningful relationships with patients, address barriers to care, and help individuals achieve better health outcomes. Care is delivered directly within the communities where our patients live and may also include telehealth services.

WHAT YOU WILL DO

• Conduct comprehensive patient assessments, including health histories, physical examinations, diagnoses, and ongoing evaluations.
• Develop and manage individualized, evidence-based care plans.
• Diagnose and manage acute and chronic health conditions within the scope of practice.
• Provide medication management, preventive care, health education, and timely follow-up.
• Identify social, environmental, and access-related barriers affecting patient health.
• Connect patients with appropriate community resources and support services.
• Collaborate with physicians, clinical leaders, and multidisciplinary care-team partners.
• Maintain accurate, timely, and complete clinical documentation.
• Deliver compassionate, culturally responsive care that builds patient trust and supports long-term engagement.
• Travel to assigned affordable housing communities throughout the Springfield market.

WHAT WE ARE LOOKING FOR

• Current national board certification in family, adult, gerontology, or adult-gerontology practice
• Active and unrestricted Massachusetts Nurse Practitioner/APRN license, or eligibility to obtain the required Massachusetts license before employment
• Active DEA registration and Massachusetts MCSR prescribing credentials, or the ability to obtain the necessary credentials
• Primary care, family medicine, adult health, gerontology, community health, or comparable clinical experience preferred.
• Experience caring for older adults and managing chronic conditions preferred.
• Strong clinical judgment and ability to practice effectively in a field-based environment.
• Excellent communication, relationship-building, and documentation skills.
• Ability to collaborate effective across clinical and operational teams.
• Reliable transportation and willingness to travel throughout the assigned Springfield-area territory.
• Commitment to serving patients who may experience barriers to traditional healthcare access.

POSITION DETAILS

• Employment type: Full-time
• Location: Springfield, Massachusetts
• Schedule: Full-time, Monday through Friday, generally 8:00 a.m.–5:00 p.m.
• Care setting: Field-based outpatient care delivered inside patients’ residences within affordable-housing communities
• Work model: Field-based outpatient care with travel throughout the assigned market and telehealth responsibilities as appropriate
• Compensation: $135,000–$175,000 annually (Actual compensation will be determined based on relevant experience, qualifications, internal equity, and other job-related considerations within the approved range.)
