Job summary

Community Healthcare Network Inc. is seeking a Family Nurse Practitioner to deliver evidence-based primary care at our Sutphin Blvd. Health Center in Queens.

This role reports to the Medical Director and works within a collaborative care team that includes nursing, health education, nutrition, social work and behavioral health. The ideal candidate will hold a valid NYS FNP license and possess at least two years of post-graduate experience in a primary/ambulatory care setting.

Qualifications

• Active NYS Family Nurse Practitioner license.
• Minimum 2 years post-graduate experience in primary or ambulatory care.
• Strong teamwork and communication skills.

Responsibilities

• Provide evidence-based primary care to patients.
• Collaborate with nursing, health education, nutrition, social work and behavioral health teams.
• Coordinate with Medical Director on patient care plans.

We are seeking a Family Nurse Practitioner for our Sutphin Blvd. Health Center.

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
