Community Healthcare Network is seeking a full-time Family Nurse Practitioner at its South Bronx Health Center, located at 94-98 Manhattan Avenue, Brooklyn, NY 11206. The role reports to the Medical Director and delivers evidence-based primary care within a collaborative care team

Ideal candidates hold a valid NYS Family Nurse Practitioner license and have at least two years of post-graduate care experience in primary or ambulatory settings. Competitive salary and generous benefits are offered.

We are seeking a Full-time Family Nurse Practitioner for our South Bronx Health Center located at 94-98 Manhattan Avenue, Brooklyn, NY 11206 .

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
