Job summary

Community Healthcare Network, Inc. seeks a Full-time Family Nurse Practitioner for the East New York Health Hub in Brooklyn, NY.

You will deliver evidence-based primary care within a collaborative team that includes nursing staff and services like Health Education, Nutrition, Social Work and Behavioral Health. The ideal candidate will hold a valid NYS Family Nurse Practitioner license and have at least two years of post-graduate/residency experience in primary/ambulatory care.

Job description

Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.

We are seeking a Full-time Family Nurse Practitioner for our East New York Health Hub located at 2581 Atlantic Avenue, Brooklyn, NY 11207 .

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
