Community Healthcare Network is seeking a Family Nurse Practitioner for the Sutphin Blvd. Health Center to provide evidence-based primary care as part of a collaborative care team in New York City.

The ideal candidate will hold a valid NYS Family Nurse Practitioner license and have at least two years of post-graduate/residency experience in a primary care/ambulatory setting. The organization invites diverse applicants and offers a competitive salary and generous benefits.

Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.

We are seeking a Family Nurse Practitioner for our Sutphin Blvd. Health Center.

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
