Job summary

Community Healthcare Network, a Federally Qualified Health Center, is seeking a Family Nurse Practitioner for the East New York Health Center in Brooklyn, NY. You will deliver evidence-based primary care and work within a collaborative care team including nursing, health education, nutrition, social work and behavioral health.

The ideal candidate will hold a NYS Family Nurse Practitioner license and have at least two years of post-graduate experience in primary care or ambulatory settings.

Qualifications

• Valid NYS Family Nurse Practitioner license.
• At least 2 years of post-graduate/ residency experience delivering care in a Primary Care/Ambulatory Care setting.
• Ability to deliver evidence-based primary care as part of a collaborative care team.

Responsibilities

• Provide evidence-based primary care to patients at East New York Health Center.
• Collaborate with nursing staff and other services to support patient care.
• Participate in care team activities and quality improvement initiatives.

We are seeking a Family Nurse Practitioner for our East New York Health Center located at 999 Blake Avenue, Brooklyn, NY 11208-3535.

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
