Job summary

HealthNet in Indiana seeks a Family Nurse Practitioner to provide high-quality clinical practice, education, and consultative services across community health centers. You will assess patients, diagnose, treat, and educate diverse populations with an emphasis on improving outcomes and access to care.

The role requires NP licensure in Indiana, Board certification, prescriptive authority, CSR/DEA licenses, and a strong commitment to CME and collaborative care in a nonprofit health system.

Qualifications

• Master's Degree in Nursing with FNP certification.
• ASN required; BSN preferred.
• National Board Certification as an FNP.
• Indiana state NP prescriptive authority certification.
• CSR and DEA licenses required.

Responsibilities

• Assess health status with health history and physical exam; perform preventive screenings and identify medical risks.
• Formulate differential diagnoses and develop treatment plans to improve health outcomes.
• Provide consultative services to health team members and community; market educational programs.
• Order and adjust medications per collaborative agreement; lead standards for nursing practice.
