Role:

Nurse Practitioner, Advanced Practice Provider

Responsibilities include:

providing effective primary care to adult Medicare patients, conducting office visits for routine and acute issues, administering wellness visits and health risk assessments, and coordinating care with other providers and specialists. The role involves building patient relationships, educating patients on health conditions, assisting with phone triage and outreach, and participating in promotional activities. Home visits and other duties may be assigned.

Qualifications:

Master’s degree in Nursing or DNP, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties, active state license, DEA license, and US work authorization.

Preferred skills:

experience in primary care (internal medicine, geriatrics, family medicine), understanding of HCC documentation, ICD-10 coding, and health risk assessments. Strong teamwork and a passion for population health are valued.

This full-time role involves 40 hours per week. The pay range is $104,442.00 - $224,952.00, depending on experience and location.
