Job summary

Role:

Nurse Practitioner, Advanced Practice Provider

Responsibilities

• Provide effective, equitable primary care to adult Medicare patients, focusing on keeping them healthy and out of the hospital.
• Conduct office visits for routine and acute issues, including annual wellness visits and health assessments.
• Collaborate with a multidisciplinary care team and coordinate with specialists and testing facilities.
• Lead population health initiatives, ensuring preventive screenings and health management.
• Assist with phone triage, patient education, outreach, and conduct home visits as needed.
• Participate in promotional activities and other assigned duties.

Qualifications & Skills

• Master’s or Doctorate in Nursing or NP certification, with active state license and DEA registration.
• Certification in Family, Adult-Gerontology, or Gerontological Nurse Practice.
• Experience in primary care, geriatrics, or family medicine is preferred.
• Knowledge of HCC documentation, ICD-10 coding, and health risk assessments is a plus.
• Strong teamwork, communication skills, and a passion for population health.
• Bilingual abilities are advantageous.
