Job summary

Personal Touch Home Health Care Services Inc. is seeking a Nurse Practitioner to provide primary care for adult Medicare patients in an outpatient setting in Augusta, Georgia.

You will work independently, performing visits, wellness assessments, and managing acute and routine issues. Responsibilities include care coordination, patient education, and participation in population health initiatives with a multidisciplinary team, plus outreach and occasional home visits to build meaningful patient

Qualifications

• Master’s or Doctorate in Nursing required and national certification in a relevant specialty.
• Active NP license and DEA registration to practice in the US.
• US work authorization and ability to work in outpatient primary care.
