AccessHealth in Greensboro, NC is seeking a Nurse Practitioner, Advanced Practice Provider to deliver primary care to Medicare adults in a collaborative outpatient setting. The role includes visits, wellness assessments, coordination of care, patient education, outreach, and potential home visits.

Job description

Role: Nurse Practitioner, Advanced Practice Provider

Responsibilities include: delivering effective primary care to Medicare adults, conducting office visits for routine and acute issues, administering wellness visits and health assessments, coordinating care with specialists and facilities, leading population health initiatives, educating patients, participating in outreach and promotional activities, and conducting home visits as needed.

Work environment: collaborative outpatient practice with a team comprising physicians, nurse practitioners, assistants, nurses, and care managers, supported by a network of specialists and hospitals.

Qualifications: Master's or Doctorate in Nursing, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioners, active state license, DEA license, and US work authorization.

Preferred skills: primary care experience, knowledge of HCC documentation, ICD-10 coding, and health risk assessments, teamwork-oriented, innovative mindset, bilingual abilities a plus.
