Health Marketing is seeking a Nurse Practitioner/Advanced Practice Provider to deliver primary care to adult Medicare patients. You will conduct routine visits, annual wellness exams, and health assessments as part of a collaborative care team.
Responsibilities include coordinating with specialists, leading population health initiatives, educating patients, and supporting outreach and home visits when needed.

Provide effective, equitable primary care to adult Medicare patients, including routine and acute office visits, annual wellness exams, and health assessments. Collaborate with a multidisciplinary team, coordinate care with specialists, and lead population health initiatives. Educate patients, assist with phone triage and outreach, conduct home visits when necessary, and participate in promotional activities. Work closely with the Center Medical Director and operational leadership.
Master's or Doctoral degree in Nursing, national certification in relevant specialty, active NP license, DEA license, and US work authorization. Preferred experience in primary care, understanding of HCC, ICD-10 coding, and health risk assessment. Bilingual skills and a passion for teamwork are advantageous.
