Job summary

Jobtailor seeks an experienced Nurse Practitioner, PA, or MD/DO to deliver remote, patient-centered care for adults with complex conditions via telemedicine in California. You will manage a patient panel, develop individualized care plans with interdisciplinary teams, and coordinate care across primary care, specialty, behavioral health, and community resources.

Job description

• Deliver exceptional remote care via the telemedicine platform
• Deliver comprehensive, patient-centered care for adults with complex medical, emotional, and social needs
• Develop and implement individualized care plans with patients, families, and interdisciplinary team members
• Coordinate care across primary care, specialty providers, behavioral health, and community resources
• Facilitate transitions of care to prevent readmissions and ensure continuity
• Diagnose, assess, and treat acute and chronic medical conditions
• Provide chronic condition management, preventive care, and acute or palliative care
• Perform in-home visits, telehealth, and clinic-based care
• Educate patients on disease management, medication adherence, and preventive health
• Address behavioral health concerns and social determinants of health
• Engage in longitudinal care and population health initiatives
• Track and document patient outcomes and participate in quality improvement initiatives
• Use population health data to identify at-risk individuals and proactively manage care
• Use telemedicine technology for video consultations, EHR documentation, and patient interactions
• Collaborate with physicians, nurses, social workers, case managers, and field-based nurses
• Participate in an on-call rotation, including evenings, weekends, and holidays; respond to urgent issues, elevate incidents, document resolutions

Requirements

• Minimum of 3 years of clinical experience with strong clinical skills, with a focus on complex care or population health
• Experience in primary care, preferably with geriatric and palliative care patients
• Proven ability to manage a patient panel and provide longitudinal care
• Comfortable collaborating with nurses and other team members in the care process
• Proficient in telemedicine technology, including EHR systems and videoconferencing platforms
• Active and unrestricted licensure as a Nurse Practitioner (NP), Physician Assistant (PA), or Medical Doctor (MD/DO) in the state of California is REQUIRED
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics is preferred
• Familiarity with telemedicine platforms and digital health tools
• Spanish language skills preferred but not required
• Must be able to physically perform essential duties including lifting 30 lbs., transporting materials, stooping, kneeling, crouching, reaching, use of hands, balancing, walking, standing, talking, hearing, and typing
