Job summary

Alignment Healthcare is seeking a qualified NP or PA to conduct in-home assessments and deliver care under the Care Anywhere program in California. You will perform 3.5 days of in-home visits, plus 1.5 days of remote clinical work, to manage health plan members and coordinate with PCPs and specialists.

Qualifications

• Required: Master’s degree from an accredited NP Program or PA program
• Active Nurse Practitioner or Physician Assistant California license
• Active Nurse Practitioner Board Certification/Physician Assistant Certification
• NPI Number
• DEA
• Valid BLS
• Valid Driver License and current automobile insurance
• Ability to communicate positively, professionally and effectively with others
• Effective problem-solving, organizational, and time management skills
• Ability to work in a fast-paced environment
• Preferred: One (1) year of prior clinical or home care experience
• Previous EMR experience preferred
• Experience caring for older adult (geriatric) patients preferred
• Ability to regularly stand, walk, sit, use hands and fingers, handle or feel objects, tools, or controls, and reach with hands and arms
• Ability to frequently lift and/or move up to 20 pounds
