Job summary

Jobtailor is seeking a Nurse Practitioner or Physician Assistant to provide home-based palliative care in the Virginia Beach area. You will collaborate with physicians, patients, and families to create and implement clinical plans of care for adults with chronic and complex conditions, with regular patient education and documentation.

Travel to client sites is required. Applicants should hold an MS in Nursing and an active NP/PA license with CPR and DEA (or eligibility).

Qualifications

• CPR certification required.
• 2 years in home-based services or hospice/palliative care preferred.

Responsibilities

• Collaborate with physicians and providers to develop complex care plans.
• Deliver clinical and non-clinical support for palliative patients at home.

• Obtain patient histories, perform exams, and order diagnostics as needed.
• Coordinate care with interdisciplinary team and medical providers.

• Travel to client sites; occasional meetings or training at office.

Job description

• Collaborate with company physicians, patients' other physicians and providers, and family members to develop complex plans of care
• Provide clinical and non-clinical support to patients in palliative care within a designated territory
• Provide services in home settings
• Develop and implement clinical plans of care for adult patients with chronic and complex conditions
• Gather patient histories, physical examinations, and diagnostics as needed
• Develop and implement treatment plans based on patients' goals of care and current conditions
• Address patients' and families' physical and psychosocial needs with interdisciplinary team support
• Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis anticipation and prevention
• Coordinate care and resources with clinical team members, physicians, and medical providers
• Maintain patient medical records and documentation
• Participate in continuing education as required by state
• Prescribe medication as permitted by state prescribing authority
• Travel to client sites or designated locations, with occasional office attendance for meetings or training

Requirements

• MS in Nursing
• Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
• Possession of DEA registration or eligibility
• CPR certification
• 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred
• Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience highly preferred
• Experience working with Electronic Medical Records (EMR) preferred
• Bilingual in English/Spanish preferred
• Satisfactory completion of a Tuberculosis test required for the Carelon Health business unit
• Travel may be required
