Jobtailor seeks a Family Nurse Practitioner (FNP) to deliver primary care across mobile and brick-and-mortar clinics in California. You will perform exams, diagnose illnesses, manage chronic conditions, and maintain meticulous EHR records while coordinating with care teams and supporting clinic operations.

Qualifications

Master's or DNP in Family Nurse Practitioner completed
Current FNP board certification (ANCC or AANPCB)
California APRN licensure or eligibility
Clinical experience in family/primary care preferred
Experience with EHR preferred

Responsibilities

Conduct physical exams and diagnose/treat acute and chronic illnesses
Take medical histories and perform physical examinations
Order and interpret diagnostic tests
Develop and manage treatment plans for chronic conditions (diabetes, hypertension, asthma)
Travel to community locations and mobile unit; work in brick-and-mortar clinic as needed
Administer vaccinations and conduct health screenings
Educate patients on prevention and healthy lifestyles
Maintain patient records using EHR; assist with clinic operations
Collaborate with healthcare providers, nurses, and medical assistants
Participate in community outreach and leadership planning

Job description

Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care
Take medical histories and perform physical examinations
Order and interpret diagnostic tests
Develop and manage treatment plans for chronic conditions including diabetes, hypertension, and asthma
Maintain accurate and timely patient records using EHR systems
Assist with opening and closing the clinic
Manage inventory, patient follow-up, insurance verification, payment collection, appointment scheduling, and cleanliness
Administer vaccinations and conduct health screenings
Educate patients on disease prevention and healthy lifestyles
Collaborate with healthcare providers, nurses, and medical assistants
Participate in community outreach activities
Participate in leadership-level planning for growth opportunities
Travel to community locations and work regularly on and around the mobile unit
Work in the brick-and-mortar clinic as needed when mobile clinic events are not scheduled

Requirements

Master's or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program
Current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification
Currently licensed or eligible for California APRN licensure as a Family Nurse Practitioner
Clinical experience in family and primary care services preferred
Experience with EHR preferred
Bi-lingual preferred
Willingness to provide world-class service to all stakeholders
Ability to travel to various community locations, often in underserved or rural areas
Ability to work some evenings or weekends, depending on the clinic schedule
Ability to work in outdoor and varied weather conditions
