Job summary

Grafton Integrated Health Network is seeking a compassionate and collaborative Nurse Practitioner to join our team at Cold Spring, Minnesota PRT. This hybrid role features a small client load and a focus on person-centered care for children with complex behavioral health, developmental, and intellectual disabilities.

Grafton Integrated Health Network is seeking a compassionate and collaborative Nurse Practitioner to join our team at our Cold Spring, Minnesota PRT. Enjoy a small patient caseload of approximately 18 clients, a flexible hybrid work schedule, and the opportunity to work in tandem with an experienced psychiatrist to provide person-centered care for children with complex behavioral health, developmental, and intellectual disabilities.

As a Nurse Practitioner, you will provide comprehensive, person-centered medical care to individuals with behavioral health, intellectual, and developmental disabilities. Provides general medical care and treatment to Grafton’s client population (children and adolescents) under supervising authority of a physician. Assesses the physical/emotional needs of individuals with developmental disabilities and psychiatric conditions. Prescribes or recommends drugs or other forms of treatment. Supports provision of a therapeutic milieu by working in collaboration with academic and residential administrators. Provides direct services as defined in the scope of practice that pertains to his/her license. Spends 80% of work hours providing direct billable services, such as physical examinations, same-day sick client visits, etc. Reports to Executive Director administratively. Practices under supervising physician.

ESSENTIAL JOB FUNCTIONS

  • Conducts admission file reviews for the organization
  • Provides admission and/or annual physicals
  • Provides same-day visits for sick clients
  • Identifiesneed for, initiates, and coordinates referrals for specialty consultations; Coordinates follow-up with external medical specialists
  • Medication Evaluation and Review – Assess clients todetermineif pharmacotherapy is warranted.Prescribeappropriate medicationfor client condition, conduct periodic medication review tomonitormedication effectiveness andpossible sideeffects, adjust medication regime according to client needs
  • Orders and interpretsappropriate laboratorystudies, x-rays, electrocardiograms, and other special examinations/testing toidentifyand assess client’s clinical problems and health care needs
  • Discuss clients with supervising physician, as scheduled
  • Attendmulti-disciplinary team meetings as needed to consult withthe treatmentteam about the course of treatment.Participates with regional team in providing leadership on treatment issues
  • Documents client files, records physical findings, and formulates plan of care based on client’s condition
  • Signs verbal orders within 24 hours of giving the order; communicates any concerns about the procedure with the assigned treatment team
  • Counsels andadvisesclients and families about physical, developmental, and psychosocial aspects of health through individual and group contacts to promoteoptimalhealth
  • Contributes to the health education of individuals and groups and applies methods designed to increase each person’s motivation to assume responsibility for his/her own healthcare.
  • Model a positive/professional leadership attitude when dealing with others in a day-to-day work situation
  • Participates as an active member of the Medical Committee
  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS

  • Requires aMaster’s degree in nursing.
  • Minimum twoyears experiencein the field.
  • Must hold a valid Minnesota nursing license as issued by the Boards of Nursing and Medicine.
  • Must posses Advanced Practice Nursing (APRN) license to practice autonomously.
  • Must hold a Professional Certification in both Pediatric or Family AND Psychiatric by examination consistent with the area of specialty (consideration may be given to individuals who hold a certification in one specialty with the ability to obtain the second upon hire).
