Grafton Integrated Health Network is seeking a Licensed Practical Nurse for PRN coverage in Virginia. You will provide nursing services, monitor client wellbeing, and support treatment plans in a collaborative setting.

The role emphasizes compassionate, evidence-based care and attention to safety in a behavioral health environment. Responsibilities include vital signs, medication administration, documentation, and crisis intervention as needed.

Qualifications

• Requires a high school diploma or equivalent; Associate degree preferred.
• Valid driver’s license.
• Valid LPN License from the State Board of Nursing.
• 1 year or moderate experience as a practical nurse in a behavioral health setting, or equivalent.
• Thorough knowledge of practical nursing techniques.
• Computer proficiency and strong communication skills.

Responsibilities

• Assesses and evaluates clients; collects vital signs and samples; monitors response to treatment.
• Follows through with treatment plans with RNs or physicians.
• Schedule appointments and attend neurology/specialty visits as needed.
• Document care in the client medical record in a timely and thorough manner.
• Administer medications as directed.
• Provide crisis intervention services and vaccines to staff.
• Participate in MDT meetings and support staff and students.

Job Summary

The LPN provides nursing services and health care to Grafton clients. Performs a variety of nursing duties associated with monitoring client’s health and general wellbeing, managing health issues and providing health care. Report to nurse manager.

Essential Job Functions

• Assesses and evaluates clients; obtains and reviews pertinent medical data; collects vital signs and specimens; monitors client’s response to treatment.
• Follows through with treatment plans while assisting RNs or physicians in providing direct care to patients
• Assists with psychological reviews; schedules client appointments with health care providers and dentists. Attends all neurology/specialty appointments, as appropriate
• Documents the administration of care in the client medical record in a timely and thorough manner
• Provides the medical perspective into client care for all Multidisciplinary Team (MDT) meetings.
• Administers medication as directed
• Serves as a resource to department staff and students and to help deliver quality client care
• Provides crisis intervention services as necessary.
• Provides hepatitis B or flu vaccines and PPD testing to Grafton staff.
• Promotes and assists in the control of communicable disease.
• Prepares and/or completes various health services forms like medication permission forms, on-call sheets, medical/dentist appointment sheets, etc.
• Works on-call duty in rotation, as appropriate; promptly responds to calls for immediate nursing services.
• Attends meetings, seminars and training as required.
• Performs other work as assigned.

Minimum Qualifications

• Requires a high school diploma or equivalent. Associates degree preferred.
• Valid driver’s license
• Valid LPN License from the State Board of Nursing
• 1 year or moderate experience as a practical nurse in a behavioral health setting, or equivalent combination of education and experience; previous medication administration
• Thorough knowledge of practical nursing techniques
• Computer proficiency, strong communication skills, ability to maintain effective working relationships with clients and other employees.

COMPENSATION

position may be filled as LPN or RN

• RN starting at $38.46 up to $51.39/hour; commensurate with experience and qualifications
• LPN starting at $30 up to $41.23/hour; commensurate with experience and qualifications.
