PopHealthCare, LLC is seeking a Full-time Nurse Practitioner to join our team in Buffalo, NY (Erie County). We are focused on reimagining healthcare delivery and are looking for innovative individuals to help us achieve our mission.

The Nurse Practitioner's role includes providing healthcare to patients in their home and collaborating with primary care providers. A $5,000 sign-on bonus and competitive pay are offered, along with opportunities for growth in a fantastic working environment.

Qualifications

• Must be Vaccinated.
• Active, unencumbered RN and NP license in the state of practice.
• National board certification in adult, family or geriatric specialties (NP's).
• Has an active DEA license.
• Current BLS for medical provider.
• Must have valid state driver license and automobile insurance.
• Strong computer skills with knowledge working in an Electronic Medical Records system.
• 2+ years' experience in care of adult, chronically ill patients preferred.
• Must be willing to travel in a 50-mile radius from primary residence.

Responsibilities

• Provide healthcare to patients in their home.
