Job summary

  • Current Nurse Practitioner license in Oklahoma.
  • Board certification in a relevant NP specialty.
  • DEA registration and wound care certification preferred.

Responsibilities

  • Perform comprehensive wound assessments and develop evidence-based treatment plans.
  • Conduct wound care procedures including debridement and NPWT.
  • Educate patients, caregivers and facility staff on prevention and care.

Job description

The Nurse Practitioner(NP) provides comprehensive, evidence-based wound care services to patients in skilled nursing facilities, long-term care facilities, assisted living communities, and other post-acute care settings. This is a highly autonomous field position requiring strong clinical judgment, exceptional communication skills, and the ability to collaborate effectively with facility staff and interdisciplinary healthcare teams. The NP is responsible for evaluating, diagnosing, treating and managing acute and chronic wounds while ensuring compliance with regulatory standards and company clinical protocols. The NP is expected to deliver exceptional patient care while fostering positive relationships with facility partners.Essential Duties and ResponsibilitiesPerform comprehensive health assessments, including medical history and physical examinations, for patients presenting with wounds.Develop individualized wound treatment plans based on current evidence-based practices.Evaluate, diagnose and manage acute, chronic, surgical, traumatic, pressure, arterial, venous, diabetic and atypical wounds.Perform wound care procedures, including dressing changes, debridement, Negative Pressure Wound Therapy (NPWT) management, skin substitute and cellular tissue product applications, incision and drainage when appropriate and biopsy procedures within scope of practice.Prescribe medications and wound care treatments as clinically indicated.Collaborate with physicians and other healthcare professionals to ensure coordination of patient care.Educate patients and caregivers about health conditions, treatment options and self-care strategies to promote healing.Educate facility staff on wound prevention, treatment protocols, pressure injury prevention and best practices.Order and interpret laboratory studies, imaging and diagnostic testing.Monitor healing progress and modify treatment plans as necessary.Ensure compliance with healthcare regulations, safety standards, infection control practices and company clinical guidelines.Complete timely, accurate and compliant electronic medical record (EMR) documentation within company-established timeframes, ensuring documentation supports medical necessity and regulatory compliance.Develop strong working relationships with nursing staff, administrators, therapy departments, and attending physicians.Communicate changes in patient condition promptly to appropriate healthcare providers.Maintain compliance with HIPAA and patient confidentiality requirements.Participate in quality improvement initiatives.Maintain current knowledge of wound care standards, CMS regulations and clinical best practices.Participate in peer review and clinical education activities.Complete all charting within company-established timeframes.Current Nurse Practitioner license in the State of Oklahoma.National board certification as an Adult-Gerontology, Family, Adult, or Acute Care Nurse Practitioner.Previous wound care experience and familiarity with Medicare documentation requirements.Current DEA registration.Current BLS certification.Wound care certification (WCC, CWCN, CWON, or equivalent) preferred or willingness to obtain.Experience in skilled nursing, long-term care, home health or hospice.Valid driver's license with acceptable driving record.Ability to travel extensively throughout assigned territory.
