Job summary

Premise Health is seeking a Family Nurse Practitioner to provide comprehensive care for client employees at our Health Center in North Phoenix, AZ. The role runs Monday–Friday, 7:15am–4:00pm, and involves exams, procedures, care planning, and patient education.

You will collaborate with physicians and healthcare teams, order tests, and ensure documentation is up to date. A strong NP background with state licenses and national certification is required.

Qualifications

• Maintains state RN licenses/certificates to practice.
• Maintains Nurse Practitioner licenses/certificates to practice.
• Maintains State Prescriptive Authority where applicable.
• Federal DEA license as required by state(s) of practice.
• State Narcotic/Controlled Substance license as required.
• Current national certification (ANCC or AANP).
• MSN or higher; transcripts if degree not clearly nursing.
• Basic Life Support certification (AHA/ARC) required; ACLS may be required.
• 3+ years of NP clinical experience in ambulatory/occupational health/family practice/ED.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs injections, suturing, wound care, and infection management.
• Develops patient care plans and documents progress.
• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests; collaborates with physicians.
• Prescribes or recommends treatments and refers to specialists as needed.
• Maintains accurate health records and supports staff training.

As a Full Time Family Nurse Practitioner, you'll provide care to client employees in our Health Center located in North Phoenix, AZ (Zip Code 85085). The scheduled hours are Monday-Friday 7:15am-4:00pm.

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
