Job summary

Urrly in Multnomah County, OR is seeking a Nurse Practitioner to perform in-home health assessments. You choose your schedule, with a minimum of 24 hours per week, traveling within Multnomah County. No meds, Rx, or clinic duties—focus on assessments and education.

Complete 6–9 visits per day, review medical history and risk factors, collect vitals and screening data, and document findings in the EMR. You will educate members on next steps and coordinate follow-ups if red flags appear.

Qualifications

• Active NP license required.
• Master's in Nursing or higher.
• Valid driverLicense and reliable car.
• Ability to work independently in the field.
