Job summary

Massachusetts Health & Hospital Association is seeking a nurse practitioner to perform in-home health assessments for adults and geriatrics, reviewing medical history, medications, social factors, and health risks to improve care quality. Expect assessments up to one hour, with referrals and education as part of the visit.

Responsibilities include ordering tests, collaborating with physicians and care teams, coordinating follow-up, and delivering services at homes and skilled nursing facilities.

Qualifications

• Master’s Degree or equivalent with NP licensure
• Board certification in adult or family health specialties
• Current BLS, ACLS, or CPR certification
• Valid driver’s license and auto insurance
• Strong computer skills and excellent communication abilities
• Preferred: 1+ year NP experience; new grads encouraged to apply
