Job summary

Community Health Care, Stow-Kent Family Practice, seeks a Family or Adult Nurse Practitioner who is passionate about patient-centered care and joining an established, supportive team dedicated to helping patients live healthier lives.

You'll perform physical exams, document histories, develop care plans, order tests, diagnose health issues, and educate patients and families. Ohio APN authorization with prescriptive authority and EMR proficiency are required.

Qualifications

• Master’s degree from an accredited nursing program.
• APN authorization in Ohio to practice in family health by the Board of Nurse Examiners.
• Prescriptive authorization.
• Knowledge of Electronic Medical Records and information systems.
• HIPAA and OSHA regulations knowledge.
• Excellent verbal and written communication skills.

Responsibilities

• Performing physical exams and patient observations.
• Recording patients' medical histories and symptoms.
• Creating patient care plans and contributing to existing ones.
• Ordering and analyzing diagnostic tests.
• Diagnosing health issues.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Educating patients and their families to manage and prevent illness and injury.
• Collaborates with physicians in managing acute and long-term medical needs of the patients.
