Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner (PRN) to care for seriously ill patients in home-based settings across our service territory. You will alleviate symptoms and align treatments with patient goals, collaborating with a multidisciplinary team for the best possible comfort and quality of life.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Nursing degree (RN/BSN) preferred.
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively

Responsibilities

• Care for seriously ill patients and alleviate burdensome symptoms in line with patient goals.

As a hospice and palliative nurse practitioner, you will care for seriously ill patients to help alleviate burdensome symptoms and align medical treatments with patients' goals and values. As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education.
