Job summary

Agape Care Group in Columbia, SC is seeking a Hospice and Palliative Care Nurse Practitioner to care for seriously ill patients at home, alleviating symptoms and aligning care with patient goals. You will collaborate with a team of clinicians to deliver compassionate, comprehensive care in the patient’s residence.

The NP will provide ongoing palliative care and hospice education, with opportunities for leadership development and professional growth within a supportive leadership structure.

Qualifications

  • A heart to serve patients and families and a passion for providing the best possible care.
  • Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
  • Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively

Responsibilities

  • Care for seriously ill patients to alleviate burdensome symptoms and align treatments with patients' goals and values.
  • Collaborate with a team of clinicians to deliver care wherever patients call home.
  • Provide hospice service and education as an active member of the medical community in your territory.
