Agape Care Group seeks a Hospice and Palliative Care Nurse Practitioner to deliver diagnosis, treatment, and comprehensive care under physician supervision, with a focus on comfort and quality of life for patients facing life-limiting illnesses.

Qualifications

  • Graduate NP with advanced practice certification and master’s/doctoral preparation.
  • Current unencumbered APRN license with NP designation and DEA prescriptive authority.
  • National NP certification in the NP specialty.
  • 2 years of chronic disease management/long-term care experience in the last 5 years; hospice/palliative background preferred.
  • Reliable transportation and ability to lift 80–100 lbs.

Responsibilities

  • Provide diagnosis, treatment, and care under physician supervision.
  • Focus on hospice and palliative care to improve patient comfort and quality of life.
  • Support families throughout the care journey with compassion and information.
  • Collaborate with interdisciplinary team to coordinate patient-centered care.

Qualifications
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
  • Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
  • Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
